Prior to interpretation, integrity of evidence must be guaranteed. This can be done and assured across the research spectrum from randomised controlled trials to pragmatic and observational studies / real world evidence. It is vital that the underpinnings of medical evidence generation are robust, particularly in real world settings where there is increasing interest. Misinterpretation may lead to inappropriate recommendations and guideline implementation which may do more harm than good.
